SpamAssassin is just a tool to score messages.
MIMEDefang glues the tool with the mail processing in a most flexible, 
because freely admin-programmable way.

Hence, think of a way to differ incoming and outgoing messages from each 
other - for instance by source IP range or whether or not the sender is 
authentificated or the %RecipientMailers hash in filter_begin()- and call 
the SpamAssassin, query the blacklists a.s.o. conditionally.

This is done by modifying the Perl code in /etc/mail/mimedefang-filter. 
There is no on/off configuration file.

BTW: If you scan messages on the submission host, to use SpamAssassin can 
slow down the message submission drastically, so that your users might 
become upset or assume something is wrong.
