As long as MimeDefang tags instead of discarding, you can also
perform per-user operations with procmail during local
delivery.  For one or two exceptions you could adjust
the scores with a filter.   Or, if you use IMAP you
can let procmail sort into folders based on other header
contents first, then the score.  That way you can
grab certain list messages first even if they happen
to be tagged as spam.
