[Mimedefang] (maybe OT) milter settings

Hi all,
	I am trying to set up Mimedefang but am stuck earlier in the
process. I have searched for 5 days now for a clear(er) explanation on how
to install milter. I am running sendmail-11.6-3 i read the libmilter
README file but i am still very unclear about how i go about enabling
milter. Can someone point me to a step by step HOWTO. Also does
sendmail-12* come with milter enabled?
