Same setup here (md-2.16, sa-2.31, sendmail-8.12.5)...

This only happens occasionally, always in pairs.  I'm not sure what
is tripping it up, and I've disabled SA checks for the moment until
I have time to look at it.  My filter for the sa check is in
filter_end(), hardly changed from the one in the example filter
script.

SA has also managed to tag as spam some _outgoing_ emails, which has
confused both senders and receivers alike :)  So I also need to stop
filtering local email for spam checks, only incoming.
