Just to make sure... Have you rebuilt sendmail.cf using make/m4?  And
then restarted sendmail?

Is mimedefang running? Try "ps -ef |grep mime"

What does the command "md-mx-ctrl histo" show?

Do you see any errors in the sendmail and/or mimedefang logs?
