Check that the message has been processed by MIMEDefang :
it should contain this header : 
X-Scanned-By: MIMEDefang 2.34 (www . roaringpenguin . com / mimedefang)

for instance (2.34 is the MIMEDefang version number, it may be something
elsein your case). 
If not check your sendmail.mc file; 

MIMEDefang by default does not bounce messages that contains .exe 
attachments. Read /etc/mail/mimedefang-filter and customize it to 
meet your requirements. 

man mimedefang-filter will tell you which functions can be used 
(action_bounce, action_discard, ...)
