Read the Milter README (libmilter/README) carefully .

Milter is officially supported only for 8.12.x. 
Performance is improved with sendmail 8.12.x too. 
There are also some bug-fixes in version 8.12.2. 
However except FreeBSD there is no packaged version 
of 8.12.2.
