Did you run "top" or alike program to check, which processes actually 
allocate the CPU? I mean the output of MIMEDefang does not necessarily 
mean that SpamAssassin is actively running at all. E.g. If you have a very 
slow hard disk, you might have a very high io/wait times, if you don't use 
a RAMdisk for the MIMEDefang spool, as well.

Did you reload MIMEDefang to be sure the changes are in effect. a.s.o.
