Because MIMEDefang runs under the "defang" user, you should also:

su defang -c 'spamassassin -p /etc/mail/sa-mimedefang.cf -t -d' < INPUTMSG

because sometimes one is hit by permission problems. I add option -x (nocreate-prefs), too.

BTW: Did you mean -D (debug) instead of -d (remove markup)?

or any other .cf file in the site-wide config dir. I have created a file 
for my own rules to easily differ it from files of the package itself.
