The paper advocating to drop "www." also agressivly mentioned this kind of
problem (I should have bookmarked it, it was fun reading):
Some people tend to think that "www.domain.tld" is a service rather than a
host, so some people tend to add the "mail.", when they are writing mail.
"ftp." if they use ftp, a.s.o. It works most of time, though.
Maybe, some failure (unknown user, server down, network error) gives the
initial kick to think that the address must be wrong and the "mail." gets
added.
