[Mimedefang] no filtering happening , no errors to be seen - how to troubleshoot?

Just to be sure, and to eleminate some of the most basic errors.
Did you generate a new sendmail.cf from that .mc file?
And if you did: is the milter included in that new sendmail.cf file?
There should be a section "MAIL FILTER DEFINITIONS" showing the
following line:
Xmimedefang, S=unix:/var/spool/MIMEDefang/mimedefang.sock, F=T, T=S:60s;R:60s;E:5m
