That's true - unless you explicitly list every valid address in an
access map, and reject everything else.  Which is exactly what was
suggested above.

Yes, you need to export that list from the destination server whenever
it changes.  Matthew gave a link to a script to use LDAP to pull the
list from an Exchange server.  Wiki entries for scripts to pull out
of different backend servers would we welcome.
