There are many, many failure scenarios.  It's not a general-purpose
anti-spam mechanism.  It's designed for two very narrow situations:

1) Giving an e-mail address to an untrustworthy Web site, because you
want information from the Web site operator, and

2) Subscribing to a mailing list without worrying about spam from
list-archive scrapers.  (In this case, the lock mechanism MUST be set
to "hold for review" rather than "reject".)

For anything else, especially personal correspondence, you should give
your real e-mail address.
