What does mimedefang.pl -features show? 
I'd guess your perl instance Isn't picking up Mail::SpamAssassin

I think the modules are "use"d in the mimedefang.pl code
    # Detect these Perl modules at run-time...
    (eval 'use Mail::SpamAssassin (); $Features{"SpamAssassin"} = 1;')
        or $Features{"SpamAssassin"} = 0;

I think your filter working with the $Features{'SpamAssassin'} checks worked since SpamAssassin was not picked up and hence the If block was ignored.

You can test my theory using
perl -MMail::SpamAssassin -e 'print "installed\n"'

note that the perl on your path and the on in the #! line of mimedefang.pl should be the same.

Also check your environment and the environment of the user running mimedefang.pl
