[Mimedefang] MD/SA and Outlook

Observations:

1.  The content preview header is making it into the message
2.  MIMEdefang is not putting it there, and SA called via MD is incapable of
changing the message
3.  SA is configured to add it

Conclusions:

A.  You are running SpamAssassin as a milter (why?)
B.  You are possibly also running SA via MD
C.  Your system is running very slowly because you do spam checking twice
