[Mimedefang] GTUBE test fails - MD + Sendmail + libmilter

MD newbie here.
 
We have successfully installed MD / Sendmail and Spamassasin.  We are using
libmilter and MD is working.  Emails come through with the 'Scanned by
Mimedefang' Header and the syslogs further confirm this.
 
However when using the GTUBE test
http://spamassassin.apache.org/gtube/
No report is written and the message is not tagged.  
 
Any ideas are appreciated.
