It's probably finding the *old* MIME-Base64 module on our @INC path.
You need to ensure that /usr/lib/perl5/site_perl is on @INC before
the usual include directory.  I thought I took care of this; could you
send me the output of:

	fgrep 'use lib' /usr/local/bin/mimedefang.pl
