[Mimedefang] Upgrade from 2.28 complete

I just want to thank everyone on this list that helped with my upgrade. 
  I've successfully updated MD 2.28 to 2.45 and SA 2.43 to SA 3.0.

Most of my problems centered around an old version of MakeMaker (caused 
problem compiling SA 2.64), a failed MIMEDefang module detection (Perl 
module MIME::WordDecoder -- "fixed" by using 
--disable-check-perl-modules), RH 8's default LANG setting (a lot of 
what CPAN tried to install failed because of this -- need to set 
LANG=en_US before starting CPAN), figuring out the real names of the 
Perl modules I needed for CPAN, and assuming CPAN would install 
MIME-Base64 correctly for RH8 (with INSTALLDIRS).
