If the mail server is down, then MIMEDefang will tempfail the mail.
If the mail server is rejecting the mail, then MIMEDefang simply passes
on whatever rejection code it got.  So if the mail server rejects with
a 4xx, so does MIMEDefang.  If it's a 5xx, then MIMEDefang rejects with
a permanent failure.
