The only thing I have left to suggest is on the machine that isn't logging,
run syslogd in debug mode and see if it is ever getting the log requests.

If it is getting the log requests, then you are logging at too high of a 
level and the messages are being discarded.

If it isn't getting the log requests, then you have a problem with either
MD's startup or mimedefang-filter. Do you have a "pargs" command on OBSD?
If so, dump the args MD is started with and see if you notice anything
obvious.
