Impossible to tell.  Is /var/spool/MIMEDefang on a RAM Disk?
Are you using the embedded Perl interpreter?  What's the maximum size of
message that you're passing to SpamAssassin?  If you pass a huge message
to SpamAssassin (more than around 100 kB), it can chew up CPU like you
wouldn't believe.
