The only time I've seen these messages is when being scanned by
something or when testing. If you telnet to your mailservers port 25
and disconnect without typing something you will see this message in
your logs.

It is usually a clear indication that someone has been scanning
you to check if the port is open, and fully completed the 3-way
handshake while doing so.
