Should be "action_add_header".

You can add "define(`confMILTER_LOG_LEVEL', `15')dnl" to sendmail.mc and
rebuild sendmail.cf and restart sendmail. This will increase the
verbosity of milter logging.

MIMEDefang processes are running, the socket is created, so that
Sendmail can use it? Check the syslog in general to find out what is
happening with MIMEDefang and Sendmail.
