Additionally, the MIMEDefang README suggests running the following as part 
of your Sendmail startup:

sendmail -Ac -q5m

This will check the client mail queue once every five minutes.  You'll 
probably want to tweak it a bit, so it's a good idea to check out the man 
page as Stefano suggested.
