[Mimedefang] mimedefang and spamassassin

Catch that in Mimedefang, by checking $Sender, and don't send
it to Spamassassin at all.  But since a sender address can be
faked, maybe test the sending IP address in $RelayAddr
also, or instead.

In detail there are many ways to do this, but they all amount to
looking for certain things with Mimedefang and using them to
exclude messages from spam_assassin_check().  The sample filter
has a test for message size.
