And you're getting "X-Scanned-by:" headers, right? (I forget if you
mentioned this or not).

If the message is spam, you should see the spamassassin headers. With
the default set up, you'll only see those scoring headers if the
message is spam.

Have you tried running with just the stock mimedefang-filter as it
comes, getting that working, then making changes?

The reports should be included in the body of the message (with those
settings).
