[Mimedefang] MUST use client-queue runner?

    Yes.  Follow Sendmail's installation guidelines.  You'll end up 
running two sendmail daemons, one that functions as an MTA, and the 
other as an MSA.  MIMEDefang uses the MSA to inject messages into the 
queue.  Basically it works like this:

    Email comes in to your server and the MTA picks it up, hands it off 
to milter, which in turn hands it off to MIMEDefang.  When MIMEDefang is 
done with its trickery, it injects the message into the MSA, which then 
delivers it to the final recipient.
