[Mimedefang] Intermittent Spam slipping through

What file have you saved the SA settings to?
/etc/mail/spamassassin/sa-mimedefang.cf gets read if it
exists, if it doesn't then MIMEDefang looks for the local.cf.

This setting won't do much as SA has no control over the mail.
You'll need to do the same trick with MIMEDefang via the
filter.

Also, for these to work you should make sure you have

$SALocalTestsOnly = 0;
