[Mimedefang] Intermittent Spam slipping through

I am seeing an occassional spam getting through which SpamAssasin
reports as exceeding my spam threshold.  My mimedefang filter is below.
It's pretty much using the stock example filter, but I added a command
to silently discard spam that exceeds the threshold.  My SA rules are
also below.  Running SA 2.55 and MimeDefang 2.36 Any suggestions?
