[Mimedefang] inconsistent sa filtering

I am running the latest versions of mimedefang and spamassassin on a RH9 box with sendmail 8.12.9. I am relaying all mail to a smart host (Exchange Server 2000). When I telnet to port 25 on the Linux box and send myself a message with some nasty words, I get the subject gets rewritten properly. When I send a message from Yahoo with the same content, nothing happens. Any ideas?
