No.  Mail is only scanned when it comes *in* via SMTP.  Outgoing or
relayed mail is not scanned.  (Locally-submitted mail is scanned if
you use the default Sendmail 8.12 setup.)

So unless you run MD on both your gateway and on the mail host itself,
mail will only get scanned once.
