Does that work properly?  I'd have thought it should be:

    while (<IN>) {
	if (defined($l)) {
	    $t = $l . $_;
	} else {
	    $t = $_;
	}
	$l = $cleaner->clean(\$t);
	print OUT $t;
    }

Otherwise, we could discard input.
