Are you doing the append_boilerplate in filter_end()?  It won't
work reliably anywhere else.

Also, adding boilerplate to HTML is hit-and-miss, because I do not parse
the HTML.  The boilerplate may very well be added, but if it comes
after the </html> tag, there's a good chance it will not be displayed.
