[Mimedefang] getting SPAM in the subject line

I've read and read, checked and double 
checked, but can't seem to get it working.

Is there a step-by-step for this somewhere I can 
read to make sure I've done everything in regards 
to getting MD to put *****SPAM***** in the subject line?
