    Okay, you lost me there.  I'm trying to match anything that has a .doc extension, and claims to be an 'application/msword', grab the $entity and shove it through 'wvText'.  'wvText' by itself wants an <input> and <output> filename.  From reading MIME::Entity I get the feeling I have to reconstruct the message myself (probably by rolling my own perl)...am I correct?
