This should be in the FAQ.  Oh, wait, we don't have a FAQ...

Check $RelayAddress, and if it's coming from one of your internal
IP's, omit spam-scanning.  If you have dial-in users who authenticate
via SMTP AUTH, you can check the $SendmailMacros{"auth_authen"} value
and skip scanning for authenticated users.  (Not sure if that's the right
macro, but you get the idea...)
